Letter To Patients

Dear Town Center Pediatrics Patients,

As of 12/31/2025 Morgan Records Management is the official custodian of medical records for Town Center Pediatrics. This website will remain active as a resource and point of contact through our designated records custodian for the duration of our record retention period — up to 21 years.

Town Center Pediatrics is excited to announce a new direction for our physicians, effective January 2026. We are grateful for the opportunity to have served your family over the past twenty years, as we pursued our dream of providing personalized care to promote the health and wellness of children in our community, As a small private practice starting in 2005, we faced an uphill battle in a market shifting toward larger organization-owned clinics. We were up for the challenge, but unforeseen changes over the last five years have made it necessary for us to take a different path forward to provide the best possible care for your family. Our physician team is heartbroken to part ways, but thankful for the time we've had together and fully support one another in our new ventures. Town Center Pediatrics will remain open until December 3I, 2025. After that, we invite you to continue your health journey with us at a different location:

Drs Gilreath, Hogarth, and Todd will now be reached at Children’s Memorial Hermann at Sugar Land in January 2026.


Notes For Patients

Your health records from this Practice will be transferred to Morgan Records Management, LLC (“Records Custodian”).

The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than seven years after the last date of services you received from this Practice, although most records will be held for at least 10 years from the date of last service, depending upon applicable state and federal record retention requirements.

Digital copies of your records can easily be requested by filling out a release form on this site—just click the "Request My Records" button to get started. There is a fee for copying and delivering records, which helps cover the cost of securely and efficiently fulfilling requests. All charges are in full compliance with applicable state and federal regulations. We look forward to assisting you and ensuring you have a smooth and secure experience accessing your records.
